Federal law enforcement just made their presence known in Bloomington, Minnesota, and they weren’t there for pizza.
On Thursday, FBI agents raided the offices of Ultimate Home Health Services, a business located in a small plaza near Old Shakopee Road East and 17th Avenue South. According to reports from FOX 9, federal agents and investigators were seen carrying boxes of evidence from the site. The business shares the strip with an Asian market, a laundromat, and a pizza shop — but this raid had nothing to do with takeout.

The entry was anything but subtle: the front door of Ultimate Home Health Services showed signs of being forcibly breached, clearly the target of the day’s operation. This came just days after the Minnesota Department of Human Services (DHS) suspended the business’s license, citing allegations of fraud.

While neither the FBI nor DHS have yet released full details, the coordinated timing suggests this wasn’t an isolated paperwork issue. DHS typically suspends a health services provider’s license only after serious findings — like billing fraud, patient neglect, or improper licensing — and when that action is quickly followed by a federal raid, it’s a clear sign something bigger is unfolding.
This case is still developing, but it fits a growing trend of federal and state crackdowns on home health care fraud, particularly in programs funded by Medicaid. Minnesota has previously been at the center of massive fraud investigations involving millions in taxpayer money funneled through bogus care providers.
